228. Koreasat, 5 Aug 95, Response Mode NA, Flight Phase 1 and 5: One of three air- ignited strap-on GEMs did not separate because of a malfunction in the separation explosive transfer system. Failure to drop a GEM motor resulted in depletion of second-stage propellants.
